Safety survey

I have developed a brief survey to measure employee perception of company safety programs. This survey is completely anonymous and available to everyone.

The purpose is two-fold. The data collected will be available to companies to use in evaluating their own safety programs and a basic rating (derived from the responses) will provide information to persons seeking employment.
